Tree of Life: CHN Health Center is part of a mixed-use development, constructing 174 affordable housing units under the City’s Inclusionary Housing program and sponsored by The First Presbyterian Church of Jamaica. The community facility leasehold will be held by Community Healthcare Network, a local FQHC, who seeks to finance a “turnkey” new-state-of-the-art health center. This center will provide primary care along with other specialty services including podiatry, dental, and behavioral health to the HRSA designated medically underserved community.
